6/28/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Anthony upon arrival to the home setting. Upon arrival to the home setting, Anthony appeared to be in a positive mood evident by his facial expressions such as a smile on the face while running up to QMHP-C and providing her with his day. He verbalized to QMHP-C he did not get in trouble. QMHP-C praised him for a good job well done and encouraging him to keep up the good work. 2. Behavior: Anthony was cooperative throughout the session and reported to QMHP-C that he really does listen to his grandmother all the time but he really don’t listen to his mother. 3. Behavior: It was noticed by QMHP-C, Anthony had a tired look on his face but switched moods once session started. Anthony’s energy increased once he got comfortable in the home, initiating a conversation with QMHP-C. 1.Intervention: QMHP-C began the session allowing Anthony discuss any behaviors that were presented since the last session that QMHP-C needed to be aware of. QMHP-C explained to Anthony that he must listen and follow the rule of an authority figure and not give a response if not asked a question. QMHP-C stated that is disrespectful to laugh and ignore adults. 2. Intervention: QMHP-C stated inquired if Anthony had been disrespectful towards his mother (reported by mother to determine if Anthony would display lying maladaptive behaviors). QMHP-C prompted Anthony cease and pay attention. QMHP-C engaged Anthony in a therapeutic activity. QMHP-C explained to Anthony the purpose of the activity in an effort to improve his behaviors. 3. Intervention: QMHP-C monitored Anthony individualized behaviors in collaboration with his mother and social/family environment. QMHP-C observed Anthony in the home in efforts to determine whether he required assistance. QMHP-C engaged Anthony in conversation regarding his recent choices, progressions, and setbacks. QMHP-C encouraged Anthony to make good choices and to complete his chores before expecting any free time. QMHP-C processed Anthony negative comments to try and manage his reactions to certain prompts. 1.Response: Anthony appeared to be receptive to QMHP-Cs’ interventions and services that were provided to him during the session. During the session, Anthony displayed fidgety behaviors laying on the floor, not listening to QMHP-C, and in a very hyperactive mood. When prompted, Anthony had the ability to calm down. 2.Response: Anthony stated that he does not intentionally disrespect his mother he just does not like some of the things that she says to him as well. During the therapeutic activity Anthony needed several redirections from QMHP-C; however, after being redirected from QMHP-C he became more focused. 3. Response: Anthony transitions smoothly and safely to session area, displaying appropriate behaviors. Anthony was seen waiting for instruction. Anthony understands that problem solving starts with thinking before acting. Anthony demonstrates problem solving by staying calm and using his senses. Anthony acknowledges his improvements and vows to stay consistent with his therapy. Anthony have been doing a better job controlling impulsive and hyperactive behaviors.
